after Gov4, i received my rewards by sending 0 Algo to an address with a note.
How can i get rewards of Gov5?

IF you are eligible for rewards through the Foundation, they will be sent directly to your wallet. No need for a note. You can check your eligibility on the Foundation governance page here:

Rewards were distributed earlier today! You should see a note in the transaction relating to gov. That would be Algorand Foundation! Cheers!
